How to Become Part of the URADD Registry
Right now, we do not know how many people in Utah have ASDs. We need to
learn about as many people with ASDs in Utah as possible to have good information.
We would like as many people with ASDs in Utah as possible to be part of the URADD
project.
The information that is collected by URADD will be protected from disclosure.
We will share information about the numbers of people with ASDs to help plan for
future school and health care needs. We will share only numbers with no identifying
information. Researchers who are studying possible causes and treatments of ASDs
and other disabilities might want to ask some people who are in the registry to take
part in their studies. URADD will not share any of your personal information with
researchers UNLESS they have your full permission to do so.
Please use this form to send us information about someone
who has an ASD. You can download the forms from this web
site. The Self-Registration Form is for parents, legal
guardians or individuals with ASDs to complete. It is five
pages long and should take no more than 30 minutes to
finish.
Forms can be sent back to URADD by fax or by mail.
The fax number is (801) 585-5723.
Your fax will go to a machine in a secure area where it could
not be read by anyone who does not work for URADD.
If you have any problems downloading the forms or if you have
any questions while filling the forms out, please call URADD at (801) 585-7576
and ask for assistance from the Project Coordinator or Epidemiologist.
