The Utah Department of Health Heart Disease and Stroke Prevention Program has funding available to fund up to two new agencies to enhance or develop and implement organizational protocols, procedures, and programs to improve control of high blood pressure and/or high blood cholesterol in Utah’s Hispanic or Utah’s frontier (6 or fewer people per square mile) populations*. Frontier counties in Utah: Beaver, Daggett, Duchesne, Emery, Garfield, Grand, Juab, Kane, Millard, Piute, Rich, San Juan, Uintah, Wayne

 

If your agency chooses Hispanic Utahns, it can be in an urban or rural area as well. If your agency chooses the frontier population, you can focus on all races and ethnicities.

 

Hispanics make up 11% of Utah’s population, and have less access to health care and health insurance coverage. Residents of Utah frontier areas have higher rates of blood pressure and heart disease then the rest of the State. High blood pressure and high blood cholesterol are major risk factors for heart disease and stroke. Both conditions can be controlled through appropriate treatment and self-management.

 

Agencies can apply for up to $8,000.00. The application is due May 28, 2008.

The Utah Department of Health (UDOH) is soliciting applications for abstinence-only educational programs in the state of Utah.  Local governments, public or private not for profit organizations, for profit organizations, state offices and agencies, units of local governments, and Indian tribal governments are encouraged to apply for contract funds.  The Utah Abstinence Education Program encourages the development of effective strategies to promote abstinence among Utah youth.

 

Target Population:  The target population for the grants will be boys and girls between the ages of nine and fourteen or Hispanic youth between the ages of fifteen and nineteen years.  Applicants can choose which population they would like to target.  Please note, if you choose to apply for the Hispanic, ages 15-19 grant, at least 75% of those receiving program instruction/services must be of Hispanic origin.

 

Project Awards: Awards will be given in the amounts of $10,000 to $70,000.  

                               

Due Date:  Applications must be received by 5:00 PM on May 30, 2008.  One signed unbound original and eight unbound copies are required.

 

Project Duration:  Awarded projects will be implemented within 30 days of the award of the grant/contract.  The Abstinence Education funding will be renewable each year for up to three years depending on availability of federal funds and project performance.   Projects will be monitored each year to determine if they are meeting goals and objectives and adhering to state and federal guidelines.  A project year runs from October 1 to September 30.
