CSHCN Clinic Eligibility

General Information

• Information and referral services are available to all families at no charge.
• CSHCN strives to provide pediatric specialty services that may not otherwise be available in a child's community such as multidisciplinary developmental clinics and rural clinics.
• CSHCN focuses on early diagnosis, chronic complex conditions, developmental evaluation and transition to adult health care.
• CSHCN works closely with the child's Medical Home (primary health care provider).
• CSHCN provides pediatric specialty care, not primary, acute or emergency care.

Eligibilty Requirements

The following criteria must be met for children to be eligible for CSHCN Clinics:

• Children must be under 18 years of age
• Children must reside in Utah

Services Provided

CSHCN provides very limited treatment for children with eligible conditions.

• Multidisciplinary clinic evalation
• Limited Diagnostic Testing
• Information and referral
• Assistance with Medicaid, CHIP and SSI applications
• Translation and interpretation for information, referral and CSHCN clinic visits
• Case management
• Family support groups
• Individual transition planning

Services Not Provided

• Respite care/child care
• Transportation
• Any medications
• Routine health care
• On-going treatment or therapies
• Care for acute illnesses
• Emergency room visits
• Insurance coverage
