Basic Principles

  • All children in Foster Care are assigned a Health Care Team member to collaborate with the DCFS caseworker to oversee the health care needs of children in Foster Care.

  • The child's primary care provider and medical home is maintained whenever possible. If the child does not have a primary provider or medical home, one will be established.

  • The child's natural parent involvement is encouraged as much as possible.

  • DCFS caseworkers and foster parents are encouraged to maintain the child's home to home record.

  • The child's immunizations are tracked and kept current while in care.

  • Partnerships between the foster parents, DCFS caseworker and the health care provider are promoted.
