All children in Foster Care are assigned a Health Care
Team member to collaborate with the DCFS caseworker to oversee the health care needs of children in Foster Care.
The child's primary care provider and medical home is maintained whenever possible. If the child does not have a primary provider or medical home, one will be established.
The child's natural parent involvement is encouraged as
much as possible.
DCFS caseworkers and foster parents are encouraged to
maintain the child's home to home record.
The child's immunizations are tracked and kept current
while in care.
Partnerships between the foster parents, DCFS caseworker
and the health care provider are promoted.
