Purchasing Patches, Decals, and Replacement Certification Cards

Certified EMS personnel may purchase certification patches, decals, and replacement cards from the Bureau of EMS. Please note that your current certification status will be verified upon ordering (i.e., only currently certified EMS personnel may purchase these items, and they may only purchase items that match their current level of certification).

  • Replacement Certification Card: $5.00

To order patches, decals, and replacement certification cards, please visit or contact the Bureau of EMS. For replacement certification cards, we recommend sending a check, along with a self-addressed stamped envelope and a current photo of yourself, but we also take orders by phone, by e-mail, and in person.
