EMS Course Coordinator Seminar

Dates, Location, and Cost

Date: August 22, 2013
Time: 6:30 pm–7:30 pm
Location: Davis Conference Center, Layton
Cost: $50

Requirements

1. Must be currently certified in Utah as an EMD, EMR, EMT, AEMT, EMT-I, EMT-IA, or Paramedic (you must submit a front and back copy of your certification card with your application).
2. Must be a current Course Coordinator.
3. Must be a currently certified EMS Instructor.
4. Recertify every two years to remain current.
5. Complete U-Train registration (see link below) and submit all required documents prior to the due date. Registrations and/or documents received after the deadline will be subject to a $25.00 late fee.
6. Incomplete registrations will be denied.
7. Documents may be e-mailed to Riki Rice at rikirice@utah.gov or faxed to 801-237-0773.

Application

Applications will be accepted until the course is full.

1. Register on U-Train (Course ID 1025961).
2. Submit supporting documentation (see instructions on U-Train course page).
