New EMS Course Coordinator Course

Dates, Location, and Cost

Date: August 15, 2008
Time: 9:00 am–12:00 pm
Location: Utah Department of Health, Salt Lake City, Utah
Cost: $40 ($65 after July 25, 2008)

Requirements

  1. Must be currently certified in Utah as an EMD, EMT, EMT-I, EMT-IA, or a Paramedic (you must submit a front and back copy of your certification card with your application).
  2. Must be a currently certified EMS Instructor for at least one (1) year.
  3. Must be the co-coordinator-of-record for a class coordinated by a current Course Coordinator.
  4. Submit a letter of recommendation from the Course Coordinator including the course number.
  5. Recertify every year to remain current.
  6. Complete U-Train registration (see link below) and submit all required documents prior to the due date. Registrations and/or documents received after the deadline will be subject to a $25.00 late fee.
  7. Incomplete registrations will be denied.
  8. Documents must be mailed or hand delivered; no faxes please.

Application

Applications will be accepted through July 25, 2008, or until the course is full.

  1. Register on U-Train (Course ID 1013155).
  2. Submit supporting documentation (see instructions on U-Train course page).
