New EMS Instructor Course

Dates, Location, and Cost

Dates: July 10–12, 2008
Location: The Red Lion Hotel, 161 W 600 S, Salt Lake City
Cost: $125
Fire Instructor I or II: $50 (you must submit a copy of your Fire Instructor certificate)

Requirements

  1. Must be currently certified in Utah as an EMD, EMT, EMT-I, EMT-IA, or a Paramedic for at least one year (you must submit a front and back copy of your certification card with your application).
  2. Must be a currently certified CPR Instructor (you must submit a front and back copy of your CPR Instructor card with your application).
  3. Submit three (3) letters of recommendation outlining and describing your EMS skills and teaching abilities.
  4. Submit documentation to verify you have fifteen (15) hours of instruction experience.
  5. Submit documentation to verify that you have provided thirty (30) hours of actual patient care within the past year prior to application (EMT, EMT-I, EMT-IA, or Paramedic only).
  6. Complete U-Train registration (see link below) and submit all required documents prior to the due date. Registrations and/or documents received after the deadline will be subject to a $25.00 late fee.
  7. Incomplete registrations will be denied.
  8. Documents must be mailed or hand delivered; no faxes please.

Application

Applications will be accepted through June 20, 2008, or until the course is full.

  1. Register on U-Train (Course ID 1012924).
  2. Submit supporting documentation (see instructions on U-Train course page).
