Recertification Practical Test Download

Before you attempt to download the recertification practical test you will need to ensure you have completed the following:

  1. Be the designated training officer for a licensed, designated, or recertification testing and training agency.
  2. Have a functioning Utah-ID account. This is the same login for POLARIS, UNIS, and many other state information sites. If you don’t have an account, click here to create one.
  3. Once you have a functioning Utah-ID you must also link your Utah-ID account to your record in the BEMS certification system. If you haven’t done that, click here.

If you have completed all of the above, login below to obtain the recertification practical for your agency.
