Vaccine Provider Training, Registration Information


Want to be a 2009 Pandemic Influenza (H1N1) Provider?

To ensure that vaccine providers are adequately trained in vaccine management prior to receipt of 2009 Pandemic Influenza A (H1N1) vaccine, we ask that you and appropriate staff review the following vaccine storage and handling information. It is designed to assist you in meeting the Utah Immunization Program requirements for training in vaccine storage and handling, packing and shipping, and inventory management.

The following list identifies the items required for certification as possible 2009 Pandemic Influenza A (H1N1) vaccination site. Each item needs to be completed for each site.

  1. Complete the online Storage Capacity Survey. To access the survey click here.
  2. Complete the Storage & Handling Training. To access the training click here. All persons responsible for handling the vaccine need to watch both online videos, "How to Protect Your Vaccine Supply" and "Top 10 Vaccine Storage and Handling Errors."
  3. Complete the Storage and Handling Quiz and fax it to the Utah Immunization Program at (801) 538-9440. Please review this training with all staff associated with vaccine management and include this valuable information in the training of new employees.
  4. Site Visit - A provider relations representative may conduct a site visit (if it has not been completed) to certify the storage facility.
  5. All information will be provided to the appropriate local health department for consideration in their vaccination plans. Should your facility be selected as a possible vaccination site, you will be contacted by your local public health department.
