Resources for Information About Your Health Plan

Not all doctors and hospitals have contracts with every managed care plan. But remember that there is a direct relationship between selecting a doctor, a hospital, and a health plan. Some consumers select a health plan with which their regular doctor participates. Consider the following when selecting a plan (or doctor or hospital):

• Ask your health plan for a listing of their network doctors and hospitals
• Read carefully the information and contract your health plan gives you regarding coverage and policies (if not provided, ask)

REMEMBER: Before seeing a specialist or receiving treatment, such as outpatient surgery, you need to check with your plan to see if you need a referral or a pre-authorization.
