Inpatient, Ambulatory Surgeries, Emergency Department Encounters and Claims Data
Public use data sets (PDS) are designed to provide general health care information to a wide spectrum of users with minimal controls. Considerable efforts are taken to ensure that individual patients cannot be identified from the PDS. Patients age, diagnosis codes, physicians specialty, and payers are grouped and several data elements are encrypted under specific conditions.
IMPORTANT: As of July 1, 2015. There are new data products available and prices on our existing products have changed.

1. Please read important data request information.
We attempt to review every request within a week of receipt by the Office. Every data request is reviewed by the Privacy Officer and Office Director and requires approval from each person.
Typically a preliminary decision on your request is made within 2 weeks after original submission. Decisions may take longer if the request needs additional information or concerns identifiable data.
For all special requests, our staff will need to assess the feasibility of the request, anticipated cost, and estimated time for completion.
Please be advised that all requests for identifiable data require additional documentation and approvals.

Research-Oriented Data Set
This restricted data set includes additional confidential data elements collected in the hospital discharge abstract and is available to researchers that have obtained Institutional Review Board and HDC approval for bona fide health care cost, quality, and access research.
Institutional License Pricing: $150,000.00 (12-Month Period, All available data products listed below)
Institutional License Information
• How long is the license? The license is for a 12-month period starting with the data specified in the agreement and ending 365 days later.
• What data is covered? All available data products from OHCS, including APCD, Facilities Data, HEDIS, and CAHPS. It includes all standard limited and research datasets and up to 250 hours of staff time to created customized data sets. New data that become available during the license period may also be added to existing uses.
• What uses are covered? This is a multi-use, multi-user license that covers approved uses by institutional users covered in the agreement. Note that each use must be approved by the HDC before use can begin. The institution is responsible to inform the HDC of any changes or additions to the list of users for each project.
The agreement includes support from one or more analysts from our team who can answer questions about the data and provide other guidance on proper use of the data.
What happens at the end of the 12 month period? Normally, we would expect the institution to renew the subscription to provide continuity for researcher access. If the institution chooses not to renew the license, there would be a six-month grace period to allow researchers to finalize any approved projects already in progress. However, after the end of the 12 month period no new data will be distributed and no new uses will be approved. Of course, individual researchers may purchase single use licenses for any needed data products to continue beyond the six month extension or to start new projects.

APCD Sample File: Public Use File: Standard, limited data sample file. Contains basic and aggregated demographic information about patients' procedures, diagnoses, and prescriptions. Multi-year beginning with 2011.
APCD Data Set: Public Use File- Standard, limited data sets. Patient, provider, and payer information. Multi-year beginning 2014.
APCD Research Data Set: Standard, limited data sets. Contains confidential data elements. Available after Intstitutional Review Board and HDC approval. Multi-year beginning 2011.

Minimal selection of 19 fields contain information about patient, principle diagnosis, procedure, hospital payer category, charges and more. County level analysis only. HEDIS and CAHPS Pricing
HEDIS: Public Use File- Standardized limited data set Healthcare Effectiveness Data and Information Set (HEDIS) indicators for Medicaid, Commercial, and CHIP health plans in Utah. HEDIS is a report of the quality of care provided by a plan to its enrollees.CAHPS: Public Use File- Standardized limited data set Consumer Assessment of Health Plan Satisfaction (CAHPS) measures enrollees' satisfaction with their health plan. The data are collected from surveys administered annual. Adult surveys are administered in even-numbered years and child survey are administered in odd-numbered years. Data are available for Medicaid, Commercial and CHIP health plans.
