Medicaid, CHIP and More:

CHIP - Children's Health Insurance Program - Provides comprehensive health insurance for the uninsured children of families who qualify.

Medicaid - Provides medical assistance to low-income families, children, pregnant women, and disabled and elderly individuals.

PCN - Primary Care Network - Provides primary preventive health coverage for uninsured adults.

UPP - Utah's Premium Partnership for Health Insurance - Provides families with a monthly premium reimbursement when a previously uninsured individual or family enrolls in an employer health plan. Qualified families will receive up to $150 per person per month.

Other Health Services

Birth and Death Certificates - The Utah Office of Vital Records is responsible for registering all vital events which occur in Utah (birth, death, marriage and divorce.) Using SILVER, you may access information and instructions for ordering certified copies of these events

Disease Reporting and Facts - Provides instructions on reportable diseases and helpful facts on diseases and health conditions of public health interest. Hundreds of diseases are listed on this web site

Primary Care and Rural Health - A health resource for Utah's rural, multicultural and underserved communities. The Office coordinates federal, state, and local efforts aimed at improving the health of Utah's rural, medically underserved, and multicultural residents.

Health Care Safety Net - We provide programs for targeted populations such as mothers and children; we also provide immunizations and assist with prenatal care where necessary.
