Utah state law requires that schools, early childhood programs, long-term care facilities (LTCs) and hospitals collect immunization information and report immunization data annually. Schools and early childhood programs required to report immunization status of currently enrolled children include: public, private, charter, parochial, early childhood programs, nursery or preschools, licensed daycare centers, childcare facilities, family home care facilities, and Head Start Programs. Long-term care facilities are required to report immunization status of residents or patients and employees and include: nursing care, assisted living facilities I & II, small care, care facilities for the mentally impaired, transitional care, and hospital swing beds. Hospitals are required to report only employee immunization data.

Reporting is also required for lab-confirmed acute and chronic cases of viral Hepatitis B in pregnant women. The Perinatal Hepatitis B Program tracks the prospective infant’s immunization and immune status to prevent infection. Adverse medical events occurring at or near the time of vaccination is voluntary and may be reported through the National Vaccine Adverse Event Reporting System (VAERS).

The Utah Immunization Program maintains two online systems for reporting immunization data: the School & Childcare Immunization Reporting System and the Long Term Care Immunization Reporting System.
