- Immunization Record Assessments will calculate the immunization rates of children in your practice.
- Immunization Assessments will help identify problems
(i.e. missed opportunities, late starts, documentation errors or problems, and reminder/recall methods).

Immunization Record Assessments can be done in your office at your convenience.

Immunization Record Assessments is a free service provided by the Utah Department of Health, Immunization Program.
