To help you find a provider, or to see the compliance history of a potential or current provider,
you can review a child care facility record, you can either search the Child Care Licensing database, or call our Salt Lake or Provo Child Care Licensing office.

You can click on the "View Licensing Record" button next to each provider's name to find information about substantiated complaint allegations
and cited findings for the past 2 years. This does not include any inspections completed in the last 30 days.
If you would like to check a provider's record prior to the last 2 years, please call our Salt Lake or Provo regional office and request a File Check
(Contact Information).
When requesting a file check, you will receive information about the provider's license, a brief explanation of our inspection and complaint
investigation process, a description of cited findings issued at inspections and complaint investigations, substantiated complaint
allegations, and when those findings were corrected. This file check does not include any inspections completed in the last 30 days.
