Initial Background Screening Form: Owners, members of the governing body, directors, employees, providers of care, volunteers, anyone age 12 years old or older who lives in a home where care is provided, and anyone who has unsupervised contact with a child in care is required to have a background screening.

New child care licenses and certificates: Initial background screening forms must be submitted with applications for new license and certificates. Fingerprint cards and the $36.50 per card fingerprint processing fee must be included with forms for individuals 18 years old and older. Fingerprint cards are available at the Child Care Licensing offices or by calling Public Safety at 801-965-4445. Individuals must clear background screenings before new licenses or certificates can be issued.

Current child care licenses and certificates: Initial background screening forms must be submitted within 5 working days of individuals beginning to volunteer, starting to work, moving into a home where care is provided, or turning 12 years old and live in a home where care is provided. If the child care license or certificate was first issued before July 1, 2013, fingerprint cards and the $36.50 per card fingerprint processing fee must be included with forms for individuals age 18 and older who have not continuously resided in Utah for the past 5 years. If the child care license or certificate was first issued after June 30, 2013, fingerprint cards and the $36.50 per card fingerprint processing fee must be included for all individuals age 18 and older. Individuals under the age of 18 who submitted initial background screening forms without fingerprint cards must submit fingerprint cards and the $36.50 per card fingerprint processing fee when they turn 18 years old. Fingerprint cards are available at the Child Care Licensing offices or by calling Public Safety at 801-965-4445.

Annual Renewal of CBS/LIS Criminal History Information and Disclosure & Consent Statement Forms for CBS/LIS Background Screening: These forms are to include the names of all owners, members of the governing body, directors, employees, providers of care, volunteers, anyone age 12 years old or older who lives in a home where care is provided, and anyone who has unsupervised contact with a child in care. Each individual whose name is listed on the "Annual Renewal of CBS/LIS" form must also be listed on the "Disclosure & Consent Statement" and must sign the Disclosure & Consent Statement next to their printed name.

Renewal child care licenses and certificates: Renewal background screening forms and disclosure statements must be submitted with renewal applications.
