Online Complaint Process

Please read all of the following information before proceeding.

Submitting a complaint online will only work if you have an e-mail program properly installed and configured on your computer. If you are accessing this website from a shared computer (e.g., using a library kiosk), or you are using a Web browser for your email, you will need to call in the complaint.

 

1. It is important for you to complete all sections of this report so your complaint can be fully evaluated.   If a particular section does not apply to your situation, simply write "N/A" (not applicable) in the space.

 

2.  Your complaint will be treated as confidential.  Under the law, complaints concerning the violation of Utah's licensing laws are not public information.

3.  If you wish to contact the Bureau of Child Care Licensing by phone, please call (801) 538-9084.  Bureau hours are Monday through Friday (except legal holidays) from 8:00 a.m. to 5:00 p.m. Mountain Standard Time.

 

4.  Please be advised that the Bureau of Child Care Licensing will not act as your attorney for purposes of settling, recovering monetary loss, or performing any other dispute resolution between you and the licensed facility whom you feel may have violated Utah's licensing law.

 

5.  Further, the Bureau of Child Care Licensing cannot perform investigations for the benefit of your personal litigation.  However, your complaint is very important to the Bureau of Child Care Licensing because it is informing Bureau of Child Care Licensing personnel of a possible violation of the licensing laws.  Such complaints are invaluable in helping to bring appropriate administrative remedies against facilities who are unlicensed or otherwise in violation of Utah law.  All complaints are given very careful attention.

 

6.  Once your complaint is reviewed, you will be contacted by a member of the Bureau of Child Care Licensing staff regarding the action the Bureau of Child Care Licensing may be able to take.
