ICF/ID Transition Program

The ICF Transition Program is a way for people living in an Intermediate Care Facility to move to another place, like a group home or other community setting. Utah Medicaid and the Division of Services for People with Disabilities invite you to participate in a phone call that will help you to learn more about living in the community. We will talk about the Transition Program in more detail and supports you can get to live safely in the community. The dates, times and call-in information are listed below. The discussion will follow a PowerPoint presentation that is available here.

During the months of September and October, the State will be conducting informational meetings about the ICF/ID Transition Program at each facility. Letters will be mailed to residents of each facility, guardians and authorized representatives letting them know the dates and times of these visits. If you are unable to attend the meetings in person, the State will also be hosting 2 conference calls to discuss the program. The dates/times of those meetings are:
