This website will provide access to the latest updates on the HCBS Transition Plan for the State of Utah as well as provide a means to sign-up for list-servs, and provide feedback on the Transition Plan, and its associated tools and publications.

Statewide Transition Plan Version 5

As a part of the State's work toward achieving full compliance with the HCBS Settings Rule, Utah has continued to progress through our State Transition Plan.

Version 5 of the plan includes information on the Provider Self-Assessment and Validation process; items/changes required for CMS approval; and the inclusion of information/changes following the receipt of feedback received from stakeholders during focus groups held in 2018.

A summary of the major changes between versions 4 and 5 of the STP highlighting new/revised content can be found in the Appendix document, pages 2 & 3 (Section A-1).

In addition, the State will be hosting an in person meeting to go over the changes in the Plan in addition to responding to questions/receiving feedback. This meeting will take place on Friday, January 25, 2019 from 1:00 pm to 2:30 pm at the Martha Hughes Cannon building, 288 N 1460 W, Salt Lake City, room 125.
