Questions? Call your local Medicaid Eligibility office for information about qualifying for Medicaid or with questions about your Medicaid eligibility. Call Medicaid Information  1-800-662-9651 for information about coverage and benefits, or if you are being billed for a medical service you thought was covered by Medicaid.

Need to change Health Care Providers?

If you live in the Wasatch front (Salt Lake, Utah,  Davis, Weber, and Morgan Counties), call your local health program representative.
If you live in Tooele or Summit County, call Medicaid Information, toll-free 1-800-662-9651. 
If you live in Box Elder or Washington County, call your local County Health Department.

Mental Health Services: For information about covered mental health services, call the mental health center listed on your Medicaid card or click here. If the center identified on your card is not correct, call your Medicaid worker at your local Medicaid Eligibility Office. 

Insurance information: The name of your Third Party Liability (TPL) insurance is on your Medicaid card. To change this information, call the Office of Recovery Services, Team 76:  Salt Lake City area 536-8798 or call toll free in Utah:  1-800-821-2237.

Is patient eligible? Use Access Now, ANSI 270, ANSI 276  or call Medicaid Information 1-800-662-9651 to find out if a patient is covered by Medicaid or PCN, the months of eligibility, co-payment/co-insurance, and "spenddown".

Questions about Managed Care: Call the plan in which the client is enrolled.  Plan is listed on the Medicaid Card, or use Access Now, ANSI 270, ANSI 276  or call Medicaid Information 1-800-662-9651.
