Show Medicaid
Card Before Receiving Medical Services!
It
is important to show each health care provider your Medicaid card BEFORE you
receive any services. To avoid problems, carry the cards listed below
with you each time you seek health care services.
Medicaid
Card: Show your Medicaid Identification card BEFORE each appointment and
BEFORE you order medical supplies, prescriptions, eyeglasses, hearing aids,
etc. Then you can find out if the health care provider accepts Medicaid
and whether the service is covered. Part of your health care may involve
other medical providers, such as lab, x-ray, and anesthesiology services.
Please remind your doctor to make sure all other medical providers accept Medicaid
and know you are a Medicaid patient. Your doctor can give the other providers
your Medicaid Identification Number so they can bill Medicaid.
Managed
Care Plan Card: If you are also enrolled
in a Managed Care Plan (MCP), the name of the MCP is printed
on your Medicaid card. The MCP may also give you a
separate MCP card with instructions. Show both your
Medicaid card and the MCP card to each medical provider.
Then you can find out if the services you will receive are
covered by the MCP.
Mental
Health Services, Pharmacy, and Dental: Your Medicaid
card states which provider you must use for mental health
services, pharmacy prescriptions, and dental services.
Even when you go to the provider listed on your card, always
show your Medicaid card FIRST! This will make sure
the provider knows to bill Medicaid and that the services
you need are covered by Medicaid. Picture
I.D. Card: When you see a health care provider for
the first time, you may be asked to show a picture identification
card as well. This is to prevent someone else from
using your Medicaid or MCP cards to obtain health care services.
Neither Medicaid nor a MCP will pay a provider for services
given to a patient who was not covered.
