Fee Schedule:

Utah Medicaid HCPCS and CPT Schedule - HCPCS and CPT codes with their Utah Medicaid payment amounts.  This is an Excel file compressed into a "zipped" file.

This file contains fees as of 9/2/2008

Note: Beginning with the 9/16/2005 version of the fee schedule, the files are created as "zip" files.  Windows XP has a built in ability to extract the zip files.  Older versions of Windows will require require an unzip program which can be downloaded free from one of the following locations.
     (1) WinZip
     (2) Simply Zip
     (3) Zipper
     (4) Info-ZIP
     (5) The Unzip Wizard
     (6) The Ultimate Zip
Historical fee schedules (prior to 9/16/2005) are self extracting files (i.e. "*.exe files") and do not require an unzip program to extract.
