Currently, PCN is only accepting applications for parents who have children under age 19 at home. Enrollment for parents will stay open until further notice.
You may apply for PCN in three ways:
• Apply online
• Print and mail a PCN application. Applications must be received during Open Enrollment
•
• Stop by your local office
If you are uninsured, but you have access to health insurance through your employer, Utah's Premium Partnership for Health Insurance (UPP) may be able to help you pay for it.
