| Health Program Representatives (HPR) work with the Department of Workforce Services to help
people eligible for Medicaid, CHIP or PCN select a health plan. If you live in Utah, Salt Lake, Davis
or Weber Counties, you must choose a health plan for your medical care. If you live in any other county
in Utah, you may have a choice of selecting a health plan or primary care provider (PCP). HPRs hold orientation classes about your benefits. During the orientation the HPR will talk to you about the health plans available in your area and help you choose one that will work best for you. |
