What is an LHD?

LHD is the acronym for Local Health Department. Every LHD provides a variety of direct public health services.

Medicaid is contracted with 7 rural LHDs:

  • Bear River District
  • Central Utah Health Department
  • Southeastern Utah District Health
  • Southwest Utah Public Health
  • Tooele County Health
  • Wasatch City/County Health

These LHDs have a partnership with Medicaid to help you choose a health plan or PCP. You can contact a HPR at any of the contracted LHDs.

Your LHD HPR can:

  • help you understand your Medicaid benefits.
  • help you understand the difference between a health plan or PCP.
  • change your heath plan or PCP.

A list of LHD phone numbers is in the Resource section of your Medicaid Member Guide or on the HPR Link of this web site. You may also contact Medicaid Customer Service 1- 800- 662-9651.
