What if I have problems with my benefits?

If you feel that a service was limited or denied unfairly, or if you are being billed for a Medicaid covered service received while you were eligible, you or your representative, have the right to question these decisions or actions and ask to have a Fair Hearing.

  • Call your health plan to talk about the problem.
  • Call the Medicaid information line at (801) 538-6155 or (800) 662-9651.
  • If there is still a problem, ask your health plan how to file a grievance or an appeal.
  • After your health plan’s final decision on the grievance or appeal, you can ask for a Fair Hearing with Medicaid. You must file for a Fair Hearing within 30 days of your health plan’s final decision.
