What is a health plan?

If you live in Utah, Salt Lake, Davis or Weber counties, you must choose a health plan for your medical care. If you live in any other county in Utah, you may have a choice of selecting a health plan or a Primary Care Provider (PCP).

A health plan is a group of doctors, clinics, hospitals and other medical experts you will use for your medical care.

Know your health plan and find out how it works. Your health plan may contact you to as about your medical needs. Your health plan must obey all federal and state laws.

The name of your health plan will print on your Medicaid card.

You must use a doctor, clinic or hospital that takes your health plan or your bills may not be paid.

  • Some health plans require you to have a PCP.
  • Your provider will know where to send the bill.
  • You have the right to receive information about your health plan each year.
The health plan will not be on your Medicaid card the first month of your eligibility and maybe not the second month. If there is no health plan listed, you can use any provider that accepts Medicaid.
