LOCATED IN THE CANNON HEALTH BUILDING  
The Office of Vital Records is located on the first floor at
288 North 1460 West
Salt Lake City UT

Directions:
There are two possible routes to the Cannon Health Building.  
Begin at the Intersection of Redwood Road and North Temple:  
1) Travel north on Redwood Road to 300 North and turn east; or  
2) Travel east on North Temple to 1460 West and turn north.  
Travel about one block on either road to arrive at the Cannon Health Building.  
•   The service window is in the main lobby of the building
•   The entrance to the main lobby is in the center of the building
•   There is parking across the street.

Open Monday through Friday.  
Hours are 8:00 am to 4:30 pm.

•   The last application for a certificate is taken at 4:30 pm.
•   The last application to change a record is taken at 4:00 pm.
