Healthy Worksite Awards Application
The Healthy Worksite Award Applications are CLOSED for 2009.
There is an application fee of $60.00 (this helps cover
some of the cost of the awards and the luncheon). Please
make checks or money orders payable to the” Utah Council for Worksite
Health Promotion,” and

To pay with a credit card, contact Luz Dominguez at the
Rocky Mountain Center for Occupational and Environmental Health at 801-581-7909. Luz
will take your credit card information over the phone. A free consulting service will be provided
to companies that desire assistance in filling out the application.
